ABSTRACT

More people have died as a result of disease transmission than have died in genuine wars. Despite being a naturally occurring disease, the great influenza epidemic of 1918 killed 20 million people globally during World War I. However, genetic engineering is employed to develop bioweapon agents. Bioweapons are weapons that involve the deliberate use of chemicals and microorganisms to cause sickness and death in people. Because biological agents are readily available and are easy to carry from one location to another, they are less likely to be detected during routine security checks and they are inexpensive to produce. This weapon is both deadly and appealing to use. This paper discusses various bioweapon systems as well as computer-based techniques used in the medical and agricultural fields to detect and counter any disease spread by bioweapons. Closures of schools in the advent of COVID-19 all around the world have affected nearly about 87% of students in different aspects of life. The importance of school life and its impact on the social and psychological well-being of an adolescent has left a deep and evident impression. As per the Indian population statistics, we embrace the largest adolescent population in the world. The COVID-19 pandemic had a significant impact on the lives of millions around the world. The authors through an exhaustive survey specially designed to relate the present mental state and well-being have analyzed the critical phase. 315 participants that consisted of both males and females take up the survey. ABSTRACT

The utility of telemedicine in healthcare has been brought to the forefront by the COVID-19 pandemic. 'SwasthGarbh' (Healthy Pregnancy) is a multi-functional, interactive smartphone application for providing antenatal care and real-time medical support to all pregnant women (especially those in rural areas and/or do not have easy access to doctors). A randomized controlled trial (n = 150) demonstrates its utility in improving the quality of antenatal care, reducing obstetric/medical complications and achieving a positive pregnancy experience. The test group (patients registered on the App) showed a significantly higher number of mean (± SD) antenatal visits (7.0 ± 1.5 vs. 5.7 ± 1.8; P<0.001) as well as better compliance with the WHO visit protocol (87.2% vs. 69.8%, P<0.001) and antenatal investigations (73.2% vs. 41.7%, P<0.001) in comparison to the control group (followed-up conventionally), respectively. Furthermore, substantial reduction in medical (38.0% vs. 55.5%, P = 0.04) and obstetric (52.1% vs. 59.7%, P = 0.36) complications during pregnancy as well as significant improvement in mean (± SD) maternal systolic BP (118.9 ± 11.8 vs. 123.4 ± 14.2 mmHg; P = 0.046), diastolic BP (76.0 ± 8.4 vs. 80.0 ± 10.9 mmHg; P = 0.02) and hemoglobin (11.5 ± 1.4 vs. 10.9 ± 1.4 g/dL; P = 0.03) parameters at delivery was observed in the test group compared to the controls, respectively. All the above mentioned positive clinical outcomes were the result of the provision of high quality antenatal care, timely detection of complications, prompt medical assistance and improved medication adherence. This is first pregnancy App that provides instantaneous access to doctor's advice and is clinically endorsed as well as credible.

ABSTRACT

Background: SARS-CoV-2 evokes vigorous humoral immune responses which includes production of virus-specific antibodies of the immunoglobulin IgM, IgG & IgA isotypes. Seroconversion & production of detectable antibodies usually occurs within 20 days of symptom onset, while the kinetics of their production is variable. IgA is the major antibody class in mucosal membranes which plays an important role in SARS-CoV-2 infections. It's response in the early stage of the disease seems to be more pronounced than IgM. Objective(s): To detect the presence of serum IgA antibody response against Spike Receptor Binding Domain & Nucleoprotein of SARSCoV- 2 in naturally infected individuals as well as vaccinated individuals. Material(s) and Method(s): Confirmed RT-PCR Covid positive serum samples were tested by in-house developed SRBD IgA ELISA & N protein IgA ELISA of SARS-CoV-2. The subjects were classified according to the post onset of disease date. Serum samples of vaccinated individuals (Covishield & Covaxin) were assessed to compare IgA response. Result(s): Our results suggest a linear trend in the level of IgA antibody response POD 8 onwards in natural infection. In vaccinated individuals Covaxin groups exhibits a prominent increase in the IgA response in comparison to Covisheld. Conclusion(s): IgA might play an important role in assessing the immune status of SARS-CoV-2 infected patients. This study suggests that IgA antibody act as a promising immunological marker for vaccine study.

ABSTRACT

Context: The effect of coronavirus disease 2019 (COVID-19) on a parturient undergoing cesarean delivery (CD) is not fully understood. Aims: To evaluate anesthetic management of a COVID parturient undergoing CD. Settings and Design: Tertiary care hospital, retrospective analysis. Methodology: Hospital case record files of COVID-19 parturients who underwent CD were reviewed with respect to clinical presentation, anesthetic technique, peri-operative course, and maternal-fetal outcome. Data Analysis: Continuous variables are reported as mean +/- SD or median (range) and categorical variables as numbers (percentages). Results: Hundred COVID-19 parturients underwent CD: Ninety-eight parturients had asymptomatic to mild clinical presentation, whereas two had a severe presentation. Raised liver enzymes, raised D-dimer, and thrombocytopenia were observed in 65, 34, and 11 parturients, respectively. Combined spinal-epidural anesthesia (CSEA), subarachnoid block (SAB), and general anesthesia were administered in 72, 26, and 2 parturients, respectively. Meantime to administration of SAB and CSEA were 23.5 +/- 3.2 min and 28.4 +/- 2.8 min, respectively. Adequate block height for CD was achieved in all parturients. Post-spinal hypotension that responded promptly to fluids and vasopressors was reported in six parturients. Postoperatively, two parturients required intensive care unit (ICU) care with one maternal mortality. None of the neonates tested positive for COVID-19. Three neonates had a low Appearance, Pulse, Grimace, Activity, and Respiration (APGAR) at 5 min with one neonatal mortality. Conclusions: Neuraxial anesthesia seems to be a safe and preferred anesthetic technique for CD in a COVID-19 parturient. The incidence of post-spinal hypotension is low and responds promptly to treatment. The course of neuraxial anesthesia and the neonatal outcome is unaffected by the COVID-19 status of the patient.

An emerging technology Internet of things is the backbone for better solution in medical science research, COVID infected sampling analysis, and device integration process. 2020 may be a year of healing not only for our mother earth, but for mankind too. It is a year of change and practice to develop ourselves against adversities. Due to the recent pandemic caused by COVID-19, many lives were affected. COVID-19 has created an adverse effect on the economy, education, mental health, and physical health of humans. It has been witnessed that despite lockdown, the death rate has increased. From several statistics, it can be observed that populations with less immunity have a higher mortality rate. This study has been performed to make a checkpoint on the factors which may be responsible for determining immunity level and based on that a prediction model may be prepared using a machine learning algorithm. This proposed work employs an IoT application to collect real-time symptom data from users to identify suspected coronavirus symptoms. IoT's sensor-based mechanism adopts for enhance capability of risk minimization specially in surgery cases compilation for COVID-19 type pandemic.

ABSTRACT

Recently, a fungal disease, "Mucormycosis,” has created a huge fury in everybody's mind. It is prevalent in immunocompromised individuals, such as people suffering from Covid 19, cancer, HIV, prolonged use of steroids, immunomodulators, and diabetes. It is caused by a group of molds called mucoromycetes found in damp soil, bread, decaying fruits and vegetables, compost piles, animal waste, and moist, dusty construction sites. It can invade the human body through several routes, such as inhalation, injury site, or food/water intake. The ICMR recommends wearing a mask while visiting dusty construction sites, wearing shoes, long trousers, a full sleeve shirt, and gloves when handling soils, moss, or manure, and maintaining personal hygiene, including a good scrub bath. This article further covers the history, pathogenesis, role of Iron, GRP78, and CotH, pathophysiology, geographical spread, and treatment of this rare but fatal disease. Overall, the mortality rates account for 54%, which is further varied depending upon the patient condition, type of fungus & body site infected, speaking of which, as reported, 96% mortality rate alone accounts for the disseminated Mucormycosis. © 2021 by the authors.

ABSTRACT

The direct and indirect mental health stressors, especially associated with the "tele-burden of pandemic” added due to the adoption of the remote learning paradigm, have led to increased online fatigue, distress, and burnout. This research aims to comprehend the perception of psychological distress experienced by Indian students placed in the new online learning setting. Subsequently, the observed symptomatology is used to predict the student's susceptibility toward developing specific psychological challenges. Primarily, a phenomenological study is conducted on 732 student participants to understand their psychological well-being during this ongoing COVID-19 crisis. Subsequently, machine learning is used to train a model with learned features from the data extracted to detect six psychological states, amusement, neutral, low stress, high stress, depression, and anxiety. Two supervised machine learning algorithms, namely random forest and artificial neural network, are used to perform the predictive analytics of psychological well-being. Experimental evaluation reports a classification accuracy of 90.4% for the random forest and 89.15% for the neural network. The qualitative research findings help foster the need to look for coping strategies involving counselors and psychologists to decrease the risk of psychological distress and preserve students' psychological health and well-being in the current setting. © 2023, The Author(s), under exclusive license to Springer Nature Singapore Pte Ltd.

This study intends to develop a fraud detection model using machine learning's semi-supervised approach. It combines supervised and unsupervised learning methods and is far more practical than the other two. A bank fraud detection model utilizing the Laplacian model of semi-supervised learning is created. To determine the optimal model, the parameters were adjusted over a wide range of values. ABSTRACT

Kawasaki disease (KD) is an acute vasculitis leading to coronary artery dilation and aneurysm in untreated patients. Etiology includes infectious triggers, autoimmune response, genetically susceptible host response, windborne agents. Diagnosis of classical KD is made based on the presence of ≥5 days of fever and presence of ≥4 of the 5 principal clinical features, which include conjunctival injection, oropharynx changes, peripheral extremity changes, diffuse erythematous rash, cervical lymphadenopathy. In addition, serum markers of acute inflammation such CRP and ESR are also elevated. Patients without sufficient clinical findings are known to have incomplete or atypical KD. Prompt diagnosis is essential, and treatment should be initiated with intravenous Immunoglobulin (IVIG). Early detection and follow-up of evolving coronary artery changes should be performed by serial echocardiograms and appropriate thromboprophylaxis should be initiated in a timely fashion. Risk stratification for long-term management is based primarily on coronary changes. Patients with coronary aneurysms require lifelong cardiology follow-up. In the current COVID-19 pandemic caused by SARS-CoV-2 virus, pediatric patients may present with atypical KD or acute toxic shock like features called Multisystem Inflammatory Syndrome in Children (MIS-C). Most of them present with myocardial dysfunction and gastrointestinal symptoms in addition to atypical KD features. Although, the true mechanism is not completely understood, the proposed hypothesis for myocardial injury is due to "cytokine storm" from the proinflammatory and regulatory T cells. Early recognition and initiation of IVIG is crucial in addition to ongoing intensive care management. Long-term outcomes are yet to be determined. © Springer Nature Switzerland AG 2021.

ABSTRACT

Background: We assess the profile of adverse events following immunization of COVID - 19 vaccination of COVISHIELD in health care workers (HCWs) in Shaheed Hasan Khan Mewati Govt. Medical college Nalhar, Nuh, Haryana, India. Method(s): The Cross sectional and prospective observational study was conducted with a period of 3 months or till the desired sample size recruited in the study with follow up period of 15 days for all those subjects who were vaccinated for covid-19 in SHKM, GMC Hospital to look for AEFI with sample size more than 300. Active surveillance was done on days 3, 6 and 9 after days of vaccination for AEFI). Any AEFI noted will be managed as per the standard guidelines. Result(s): We present the results of an interim analysis of 400 patients out of total 550 participants with 244(61.00%) male and 156 (39.00%) female participants respectively. AEFIs following first dose were reported in 400 participants and 269 participants after second dose. Fever was the major AEFI with 150(37.50%) after first dose and 78(28.99%) after second dose respectively. In the study age wise AEFIs percentage of participants were also calculated. In the present study different systemic diseases percentage also calculated. Conclusion(s): The AEFIs associated with the COVISHIELD (ChAdOx1 nCoV-19), the COVID-19 vaccine injected in hospital health care workers is found to be safe for use in except for a few minor reactions.Copyright © RJPT. ABSTRACT

OBJECTIVE: To determine the clinical manifestations, risk factors, treatment modalities and maternal outcomes in pregnant women with lab-confirmed COVID-19 and compare it with COVID-19 negative pregnant women in same age group. DESIGN: Multicentric case-control study. DATA SOURCES: Ambispective primary data collection through paper-based forms from 20 tertiary care centres across India between April and November 2020. STUDY POPULATION: All pregnant women reporting to the centres with a lab-confirmed COVID-19 positive result matched with controls. DATA QUALITY: Dedicated research officers extracted hospital records, using modified WHO Case Record Forms (CRF) and verified for completeness and accuracy. STATISTICAL ANALYSIS: Data converted to excel files and statistical analyses done using STATA 16 (StataCorp, TX, USA). Odds ratios (ORs) with 95% confidence intervals (CI) estimated using unconditional logistic regression. RESULTS: A total of 76,264 women delivered across 20 centres during the study period. Data of 3723 COVID positive pregnant women and 3744 age-matched controls was analyzed. Of the positive cases 56·9% were asymptomatic. Antenatal complications like preeclampsia and abruptio placentae were seen more among the cases. Induction and caesarean delivery rates were also higher among Covid positive women. Pre-existing maternal co-morbidities increased need for supportive care. There were 34 maternal deaths out of the 3723(0.9%) positive mothers, while covid negative deaths reported from all the centres were 449 of 72,541 (0·6%). CONCLUSION: Covid-19 infection predisposed to adverse maternal outcomes in a large cohort of Covid positive pregnant women as compared to the negative controls.

ABSTRACT

Background Online learning can play a vital role in the process of teaching and learning during Corona Virus Disease 2019 (COVID-19) pandemic. However, learners' satisfaction is extremely important in effective implementation of the online learning, especially at institutions where it is newly adopted. Objective To assess satisfaction towards online learning and its predictors among students at Chitwan Medical College, Bharatpur. Method A web-based cross-sectional survey was undertaken among 434 undergraduate and postgraduate students from various academic programs who had participated in the online classes started during this COVID-19 pandemic. A structured questionnaire consisting of 31 items (5-point Likert scale) covering four major student satisfaction domains (learners' dimensions, technological characteristics, instructors' characteristics and course management and coordination) was distributed to the students using Google Form. Result More than half (53.5%) of the students were satisfied with the online learning, while 29.7% gave neutral views. Bivariate analyses found that all four domains scores were positively correlated with each other as well as with the students' overall satisfaction towards learning. In multivariate analysis, female gender [aOR: 2.72, p = 0.013], WiFi as internet modality for learning [aOR: 3.36, p = 0.001) and learners' dimension score [aOR: 1.27, p<0.001] were the significant predictors of students' satisfaction. Conclusion Although recently adopted, the satisfaction of the students towards online classes appears good, and prioritizing the identified predictors and working on the weak links could assist in enhancing students' satisfaction and better outcomes. The great use of telecommunication technology propels new healthcare system of telemedicine through which diagnosis as well as treatment can be done in the remote areas. The ancient Greek language explain the terminology of telemedicine in the phrase of distance healing. As per WHO, Telemedicine is the delivery of health-care services, where distance is a critical factor, by all health-care professionals using information and communications technologies for the exchange of valid information for diagnosis, treatment and prevention of disease and injuries, research and evaluation, and the continuing education of health-care workers, with the aim of advancing the health of individuals and communities. Historically the concept of teleconsultation was evolved in the first half of twentieth century when the data of ECG was communicated through telephone lines, this can be traced as first evidence of this unique healthcare system. Further the introduction of electrical system of telegraph as well as evolution of telephone revolutionized this system of healthcare. when the Technology of telemedicine help both patients as well as service providers in multiple ways involving physicians, surgeons, pharmacists, paramedical staff, IT and electronics engineers, government, hospitals and end user public Location is now a days no problem and therefore there is no limitation of the availability of healthcare facilities to such location or remote location. The biggest role in such development is played by the communication technology which may provide healthcare services to every nook and corner of the location. It can decrease the health staff pressure because in India WHO guidelines ask to maintain the ratio 1:1000 of doctor and Indian public compared to present 0.62:1000 ratio of doctor and public. The great advantage of this system is that in case of epidemic or pandemic like COVID 19 Telemedicine can keep the health staff are well general public free from contagious infection (COVID-19). There are a number of networking communication modes that can be applied, which may improve the patient compliance,dosage regimen can be managed in better fashion thus increase the longevity of person life. Disasters management during pandemics present unique challenges which can be addressed effectively as happened during the lockdown. ABSTRACT

Case Report: A 4-year-old African American male presented to an outside emergency department (ED) following sudden inability to move left upper extremity. Past medical history was unremarkable and routine vaccinations were up to date. Radiograph of affected extremity ruled out fractures and patient was discharged to follow up with primary care physician. Two days later mother brought him to our ED due to persistent left upper extremity paralysis, poor appetite, and subjective fever. On exam his left arm was warm and tender to dull and sharp touch;he had definite loss of active movement, hypotonia and absence of deep tendon reflexes. The patient had winging of left scapula and could not shrug left shoulder. MRI of cervical and thoracic spine showed enlargement of spinal cord from C2-C6 level with gray matter hyperintensity, slightly asymmetric to the left. Laboratory studies showed leukocytosis (14 000/mcL) and CSF studies showed pleocytosis of 89 WBC/mcL (93.3% mononuclear cells and 6.7% polymorphonuclear cells), 0 RBCs, normal glucose and protein, and a negative CSF meningoencephalitis multiplex PCR panel. Due to high suspicion of demyelinating or autoimmune condition he was treated with high dose steroids and IVIG. Subsequently neuromyelitis optica was ruled out as aquaporin-4 receptor antibodies (AB) and myelin oligodendrocyte glycoprotein AB were normal. CSF myelin basic protein and oligoclonal bands were absent ruling out demyelinating disorders. CSF arboviruses IgM and West Nile IgM were negative. He showed minimal improvement in left upper extremity movement but repeat spinal cord MRI one week later showed improved cord thickness with less hyperintensity. Respiratory multiplex PCR was negative including enteroviruses. Repeat CSF studies after IVIG showed increased IgG index and IgG synthesis suggestive of recent spinal cord infection, consistent with acute flaccid myelitis (AFM). Pre-IVIG blood PCR was invalid for enteroviruses due to PCR inhibitors found in the sample. Blood post-IVIG was negative for mycoplasma IgM, West Nile IgM, and arboviruses IgM. Enterovirus panel titers (post-IVIG) were positive for coxsackie A (1:32), coxsackie B type 4 (1:80) and 5 (1:320), echovirus type 11 (1:160) and 30 (1:80) as well as positive for poliovirus type 1 and 3. These titers could not distinguish acute infection from patient's immunity or false-positives as a result of IVIG. He was discharged with outpatient follow-up visits with neurology, infectious disease, occupational and physical therapy, showing only mild improvement after discharge. Discussion(s):With the anticipated resurgence of AFM after the peak of COVID-19 pandemic, our case illustrates the need to consider this diagnostic possibility in patients with flaccid paralysis. It is important to remember CSF IgG synthesis is not affected by IVIG. In addition when treatment plans include IVIG, appropriate samples should be collected before IVIG to facilitate accurate work-up for infectious diseases. As per WHO, Telemedicine is the delivery of health-care services, where distance is a critical factor, by all health-care professionals using information and communications technologies for the exchange of valid information for diagnosis, treatment and prevention of disease and injuries, research and evaluation, and the continuing education of health-care workers, with the aim of advancing the health of individuals and communities. Historically the concept of teleconsultation was evolved in the first half of twentieth century when the data of ECG was communicated through telephone lines, this can be traced as first evidence of this unique healthcare system. Further the introduction of electrical system of telegraph as well as evolution of telephone revolutionized this system of healthcare. when the Technology of telemedicine help both patients as well as service providers in multiple ways involving physicians, surgeons, pharmacists, paramedical staff, IT and electronics engineers, government, hospitals and end user public Location is now a days no problem and therefore there is no limitation of the availability of healthcare facilities to such location or remote location. The biggest role in such development is played by the communication technology which may provide healthcare services to every nook and corner of the location. It can decrease the health staff pressure because in India WHO guidelines ask to maintain the ratio 1:1000 of doctor and Indian public compared to present 0.62:1000 ratio of doctor and public. The great advantage of this system is that in case of epidemic or pandemic like COVID 19 Telemedicine can keep the health staff are well general public free from contagious infection (COVID-19). There are a number of networking communication modes that can be applied, which may improve the patient compliance,dosage regimen can be managed in better fashion thus increase the longevity of person life. Disasters management during pandemics present unique challenges which can be addressed effectively as happened during the lockdown. This review focuses on the management of Novel Corona Virus with antiviral drugs and antibiotics and therefore the worldwide dissemination of COVID-19 has been accompanied by increased use of antibiotics, according to this review, which focuses on the therapy of Novel Corona Virus with antiviral medicines and antiviral. This is linked to COVID-19 patients' priority of viral infections. In low-and middle-income countries, identifying viruses is difficult because to a lack of medical or cheap infrastructure that is easily accessible and inexpensive among diseases and pathogens. The possibility of COVID-19 spreading has increased public awareness of the need of antibiotic management systems, as well as infection control and control measures that can minimize microbial load. In underdeveloped nations, these measures are commonly employed. During the COVID-19 pandemic, studies were conducted as a test for worldwide antibiotic resistance. Respiratory problems are being blamed on the Novel Corona Virus that Include pneumonia, colds, sneezing and coughing, and other respiratory diseases. Humans are infected with the Coronavirus by airborne droplets. The World Health Organization has warned against visiting public areas and avoiding close contact with an infected individual. First, on December 31, 2019, the Coronavirus (2019-nCoV) was separated from the Wuhan market in China, resulting in the COVID-19 pandemic of extremely complicated viral illnesses. Patients with risk factors are more prone to develop secondary infections, which necessitate the use of antibiotics. Attempts to duplicate the medication, on the other hand, raised knowledge of the antibiotics' significance beyond infection management. Antiviral, immunomodulatory action, and unique pharmacokinetic profile of antibiotics play a significant part in the therapy of pneumonia;other benefits include cardiac safety, improved lung tissue access, and possible antiviral, and immunomodulation, but some adverse effects by usage. SARS-CoV-2 has generated an epidemic of the highly infectious new coronavirus 2019 (COVID-19), which poses a severe public health concern.
